I request that my dog(s) be admitted to The Backyard Pack dog daycare. In accepting my dog(s) into dog daycare, I agree that

The Backyard Pack will rely on my understanding and agreement to the provisions stated below. I understand and agree.

 

I acknowledge and understand that The Backyard Pack and its employees agree to provide services stated in this contract in a reliable and trustworthy manner. In consideration of these services and an express condition thereof, the client expressly waives any and all claims against The Backyard Pack or its employees, unless arising from gross negligence on the part of The Backyard Pack. I agree to notify The Backyard Pack of any concerns within 24 hours of returning home.

 

I acknowledge and understand that participating in The Backyard Pack presents a certain level of risk involved with being in the presence of dogs that may include, but not limited to, injuries to people, property, and animals resulting from dog fights, dog bites, or the transmission of diseases. The Backyard Pack shall not be held responsible for pets that bite or suffer an accidental death.

 

I acknowledge and understand that participating in The Backyard Pack presents health risks should my dog attend dog daycare without the required vaccinations and should there be any indication that my dog(s) has not received required vaccinations, I will be contacted and requested to pick up my dog immediately from The Backyard Pack. The Backyard Pack is not liable for health problems/cost associated with infections caused by unvaccinated dogs.

 

I acknowledge and understand that I the client will be required to reimburse The Backyard Pack for all costs (including, but not limited to medical care and lost wages) associated with contracting any ailments while exposed to my dog.

 

I acknowledge and understand that acutely ill animals or those with uncontrollable medical conditions will not be permitted to attend The Backyard Pack.

 

I acknowledge and understand that participating in The Backyard Pack may cause separation anxiety caused by, but not limited to, attendance at an unfamiliar location away from owners.

 

I acknowledge and understand that participating in The Backyard Pack activities may cause sore muscles, sore joints, and fatigue to dogs not used to the level of activity provided. Dogs that are not exposed to outside play may experience sore paws, blisters, bruises, or abrasions on the feet.

 

I acknowledge and understand that participating in The Backyard Pack may cause my dog(s) to get dirty or matted. Any dog(s) with long toenails will be requested to be trimmed prior to next attendance. Any dog(s) with fleas will not be permitted to attend daycare until proof of flea preventative is provided.

 

I acknowledge and understand that any behavior deemed inappropriate and/or dangerous by my dog(s) by The Backyard Pack may result in the dismissal of my dog(s) from the program. In severe cases, I may be required to pick‐up my dog immediately regardless of reservation time.

 

I acknowledge and understand that The Backyard Pack will not walk unruly or untrained dogs or dogs that choke themselves on their leash. All dogs must be walked on a leash, no exceptions.

 

I acknowledge and understand that by admitting my dog(s) into The Backyard Pack, the staff relies solely on information provided by me in regards to the health, vaccinations, and previous demonstrations of aggressive or threatening behavior exhibited toward any animal or person by my dog(s).

 

I acknowledge and understand that The Backyard Pack does not accept aggressive animals. I, the client, agrees to be responsible for all costs (including, but not limited to, medical care, attorney fees, etc.) if my pet should bite another person or animal.

 

I acknowledge and understand that any dog(s) involved in an incident will be examined for visible wounds/health issues; however, the staff of The Backyard Pack are not trained veterinarians and will not be liable for the location, treatment, diagnosis, or prognosis of any injuries incurred while on the premises. If my dog(s) is involved in an incident at The Backyard Pack I will be informed of the incident, but I am responsible for examining my dog(s) for issues and will seek treatment for my dog(s).

 

I acknowledge and understand in the case of an emergency, The Backyard Pack may be required to transport my dog to a medical facility thereby exposing my dog to the possibility of unforeseen accidents. No dogs will be transported without permission from the client.

 

I acknowledge and understand that the location of The Backyard Pack may be independently owned and not liable for incidents involving The Backyard Pack.

 

I acknowledge and understand that by signing this agreement I have authorizing the staff of The Backyard Pack full power of decision making in regards to the care and well‐being of my dog(s). Should a medical emergency arise, I agree that the staff of The Backyard Pack can and will make any necessary decisions concerning medical treatment and choice caregiver with a maximum amount of $100 and I will be responsible for reimbursement of all costs associated with the treatment.

 

I acknowledge and understand that I am required to pay for all services at the end of each day, including additional fees assessed based on the fee structure. Any fees unpaid will be forwarded for collection and I will be responsible for collections and legal fees incurred.

I acknowledge and understand that a $35 fee will be assessed on all returned checks. All fees are due promptly upon rendered services and any additional fees will be required prior to making a new reservation.

 

I acknowledge and state that I have read and fully understand The Backyard Pack Enrollment Acknowledgement and confirm that all information provided by me is true to the best of my knowledge.
